Working Together to Improve COPD Care in New Hampshire
The New Hampshire COPD Plan is a statewide roadmap designed to increase awareness, improve prevention and diagnosis, expand access to treatment and support, and strengthen collaboration among organizations working to improve the lives of people affected by chronic obstructive pulmonary disease (COPD).
COPD is a serious and often underrecognized lung disease that affects thousands of New Hampshire residents. Addressing COPD requires collaboration among healthcare providers, public health professionals, community organizations, people living with COPD, caregivers, and other partners.
Through coordinated efforts, the NH COPD Plan works to improve COPD awareness, strengthen care, and ensure individuals and families have access to the resources they need.
NH COPD Plan Priorities
The NH COPD Plan brings together partners through three key focus areas:
Public Awareness & Education
Increase awareness and understanding of COPD across New Hampshire by providing education about:
- COPD risk factors
- Symptoms and early diagnosis
- Treatment options
- Prevention strategies
- Resources available for individuals and families affected by COPD
Diagnosis, Management & Patient/Family Support
Improve outcomes for people living with COPD by promoting:
- Earlier and more accurate diagnosis
- Access to appropriate treatment
- Support for self-management
- Patient and family education
- Connections to community resources and support programs
Data Surveillance & Evaluation
Improve understanding of COPD in New Hampshire by:
- Strengthening the collection and reporting of COPD-related data
- Identifying areas of need
- Guiding public health efforts
- Helping direct resources where they are most needed
